At the handwashing sink on the line there was no handwashing cleanser at the other handwashing sink in the kitchen there were no paper towels at the handwashing sink in the bar area there were no paper towels; handwashing sinks should be fully stocked at all times; corrected on site after the observations; and staff were asked to wash hand while the health inspector observed after the handwashing sinks were restocked.

The person in charge was unable to answer questions about food safety temperatures (hot holding cold holding minimum internal cooking temperatures for proteins) was not able to answer questions about illness policy (reportable symptoms and illnesses); provide a person in charge that is able to answer questions about food safety illness policy and sanitization.

Observed multiple shellstock tags for July and June not filled out with last date sold or used; retrain staff to enter date for every shellstock tag as last is sold or used.

Multiple tiles in the kitchen area are damaged; insulation around pipes above the three compartment sink is damaged multiple baffles are missing or damaged in the cook line hood; two reach-in refrigerators are leaking water into buckets on the floor repair or replace the broken equipment.

Upon reinspection at the high temperature warewashing machine the PSI gauge did not go above 10 PSI; repair or replace plumbing. Ecolab was on site during the reinspection fixing the high temperature warewashing machine.

At the labeled prep sink no indirect connection to the waste water system observed; have a licensed plumber pull proper permits and install an air gap at the prep sink.
