Upon reinspection a pest control report from 6-2-2026 states there were 2 captured mice and observed small dark droppings in the boiler room floor and ledges. Have your pest control operator perform another service clean and sanitize the boiler room of droppings; the IPM report should show no activity and no evidence of pests.

Multiple cold holding units observed without internal thermometers; provide internal thermometers for all cold holding units.

No written procedures to clean up a vomit and diarrhea event presented during the inspection; provide.

Location uses quats sanitizer for food contact surfaces from an auto-mixer but could not produce a testing device for quats; provide testing device for any sanitizer solutions used at the establishment.

The pipes below the three compartment sink leaks water onto the floor when in use; repair or replace the pipes so they do not leak when the three compartment sink is in use.

At the front entrance below the Foxhole lighted signage there is a gap between the door and frame where daylight is visible when the door is closed; repair or replace the door so that no gaps exist to prevent pest entry.

In the rear area where the prep sinks is employees store soiled uniforms shoes and personal belongings on top of the sink and over food prep areas; provide a designated area where employees can store uniforms and belongings without posing a contamination risk to food and food contact surfaces.

The person in charge states that the location washes produce in the three compartment sink when there is a properly installed prep sink in the establshment but the area is being used to store employee belongings and clothes; the prep sink should be used to wash produce not the three compartment sink.
