Observed mold- like substance inside the ice machine. Clean to remove mold like substance.

Observed 2 holes in the wall underneath the high temp dishmachine. Repair both holes.

Observed grease on the hood filters. Hood sticker is good. Clean filters to remove grease build up. Observed dust in the kitchen ceiling vents. Clean to remove dust. Clean air filters.

No vomit & diarrhea kit- Provide . Procedures and supplies will be sent over with this report.
***Backflow Prevention (P)
Observed no air gap under the 2nd prep sink. Contact licensed plumber to install proper air gap. Observed leak coming from the diposal in the warewash area. Repair leak and remove stagnant water from the floor.
